Me, like most folks with any condition, have to understand how the body will react to weather conditions. Knowing your body and it's limitations can't be stressed strongly enough. For example: due to my neurological deficits, I don't perspire very much. That means my body's cooling system isn't operating. So, I have to be extra cautious and drink cold fluids, stay out of the hot sun for any length of time, splash or spray my body with water, use sunscreen to avoid the burn, go out earlier or later in the day, be hyper aware of how I feel.
Also, as I get more active outside, I need to eat healthy foods to maintain my stamina. I have had my blood sugar crash a few times. It is terrifying to feel your body start to shut down. Doesn't really make your companions eager to include you in the plans, either.
However, once again, understanding your body, whether you are disabled or not, and how you will react to certain conditions will enable you to get out and "blow the stink off".
